Victims of Family Violence Notification Forms
These are the forms supplied to victims of family violence. Family violence includes both physical, sexual, and verbal violence. For violence to be considered family violence, it must fall under the legal definition of family violence. These forms contain helpful information for victims seeking assistance in their particular situation. Information in these forms notify the victim of their rights under Texas Law as a victim of family violence. These forms contain the wording as follows:
- "Notice to adult victims of family violence."
- It is a crime for any person to cause you any physical injury or harm EVEN IF THAT PERSON is a member of YOUR FAMILY OR HOUSHOLD.

You have the right to:
- ASK the local prosecutor to file a criminal complaint against the person committing family violence; and
- APPLY to the court for an order to protect you (you should consult a legal aid office, a prosecution attorney, or a private attorney.)

If a family or household member assaults you and is arrested,
you may request that a magistrate.s order of emergency
protection be issued. Please inform the investigation Officer
if you wan an order for emergency protection. You need not be
present when the order is issued. FOR EXAMPLE, THE
COURT CAN ENTER AN ORDER THAT:
- the abuser not commit further acts of violence,
- the abuser not threaten, harass, or contact you at home,
- the court can direct the abuser to leave your household; and
- establishes temporary custody of the children and directs the abuser not to interfere with the children or any property.
A VIOLATION OF CERTAIN PROVISIONS OF THE COURT ORDERED PROTECTION (such as (1) and (2)) IS A CRIMINAL OFFENSE.
